R. Nachmani asks how to distinguish ++ from +++. Perhaps the question assumes something false.
These modifiers are not objective measures. They are self-assessments relative to the author's own reference frame. My ++ may be your +. Neither is wrong.
The specification cannot calibrate across people. It can only provide vocabulary for people to calibrate themselves.
