Subject: Can certificates be revoked?

This is the uncomfortable question.
A certificate of conversion says: this person appeared before the beit din, immersed in the mikveh, accepted the mitzvot, three rabbis witnessed. These things happened. They cannot un-happen.
Can the conversion be revoked? The Rabbinate sometimes refuses to recognize conversions performed by certain batei din. But they do not claim the immersion did not occur. They claim it was insufficient.
What does revocation mean for an attestation of completed procedure?
